How Long Does a Lip Flip Last? Setting Realistic Expectations

One of the most common questions patients ask about a lip flip is whether the results will last as long as Botox® in the forehead or between the brows. Usually, they do not.

A lip flip is intentionally subtle and uses a very small amount of neuromodulator in one of the most active areas of the face. For many patients, the most noticeable effect lasts approximately six to eight weeks, although some may continue to see a subtle improvement for two to three months. Individual results depend on the product used, the dose, facial anatomy, muscle strength, metabolism, and how the lips move during normal daily activities.

Understanding why a lip flip may wear off relatively quickly can help patients decide whether the treatment fits their goals and maintenance preferences.

What Is a Botox® Lip Flip?

A lip flip is a nonsurgical treatment that uses a small amount of a neuromodulator, such as Botox®, Dysport®, Jeuveau®, or Daxxify®, to subtly change the position of the upper lip.

The injections are placed precisely around selected portions of the orbicularis oris, the circular muscle surrounding the mouth. This muscle helps control movements involved in speaking, eating, drinking, smiling, and pursing the lips.

Neuromodulators temporarily reduce communication between the nerves and the treated muscle fibers. When the superficial portion of the upper lip muscle relaxes slightly, the upper lip may roll outward, allowing more of the pink portion of the lip to show. The result is typically a subtle improvement in upper lip visibility rather than a significant increase in size.

A lip flip is an off-label use of an FDA-approved neuromodulator. This means that while the medication is FDA approved for certain cosmetic treatment areas, the specific use around the upper lip has not been separately approved by the FDA.

How Long Does a Lip Flip Usually Last?

Most patients should expect a lip flip to last for a shorter period than neuromodulator treatment in areas such as the forehead, crow’s feet, or frown lines.

A noticeable improvement may last approximately six to eight weeks for some patients. Others may see an effect closer to two or three months. The change gradually fades as nerve signaling returns and the upper lip muscle regains its normal strength and movement.

Dr. Tina West has previously noted that neuromodulator treatment in the upper lip does not tend to last as long because the dose must remain small. She has also published research in the peer-reviewed journal Dermatologic Surgery examining the relationship between neuromodulator dose and duration. A shorter-lasting result does not necessarily mean that the treatment was unsuccessful. It often reflects the conservative dosing required in an area responsible for several important facial functions.

Why Does a Lip Flip Wear Off Faster?

The lips are rarely at rest. The orbicularis oris contracts throughout the day while talking, chewing, drinking, smiling, applying lip products, and making facial expressions.

The dose used for a lip flip must also be carefully limited. Too much neuromodulator could excessively weaken the muscle and temporarily interfere with normal lip function. Since the treatment uses a small dose in a highly active muscle, the visible effect may fade sooner than it does in other facial areas.

Patients may see advertisements stating that a particular neuromodulator can last three, four, or even six months. Those duration estimates are generally based on studies involving FDA-approved treatment areas, such as the muscles between the eyebrows. They should not automatically be applied to a lip flip.

The treatment area, injection pattern, dose, muscle strength, and desired degree of relaxation all affect how long the result lasts.

When Will the Lip Flip Start Working?

A lip flip does not create an immediate change. Some patients begin to notice the upper lip relaxing within three to five days, but the full result may take approximately 10 to 14 days to develop.

The lip may feel slightly different as the muscle begins to relax. This sensation generally becomes less noticeable as the patient adjusts to the change.

Patients should wait until the treatment has fully developed before deciding whether they like the result or discussing future adjustments. Adding more product too early may over-relax the muscle and affect lip movement. Neuromodulator results commonly continue developing for up to two weeks.

What Can a Lip Flip Realistically Accomplish?

A lip flip can create a small increase in visible upper lip height, particularly when the lip tends to curl inward or appear thinner while smiling. It may also help soften the appearance of certain vertical lines caused by repeated muscle movement.

However, a lip flip does not add physical volume to the lips. It does not provide the same degree of projection, structure, hydration, border definition, or asymmetry correction that may be achieved with a carefully selected hyaluronic acid filler.

Patients looking for a very subtle change may appreciate a lip flip on its own. Those seeking more noticeable volume or definition may be better suited for lip filler. In some cases, a conservative combination of neuromodulator and filler may provide a more balanced result.

The appropriate treatment depends on the patient’s natural anatomy, how the lips move, and what the patient hopes to change.

Does a Lip Flip Require Maintenance?

As a lip flip is temporary, maintenance is necessary to preserve the effect. However, the visible result wearing off does not automatically mean that another treatment should be performed immediately.

The appropriate interval depends on the neuromodulator used, the dose, the patient’s response, and the provider’s clinical judgment. Repeating treatment too frequently or increasing the dose simply to extend the result may affect lip function or create an unnatural appearance.

Are There Any Trade-Offs?

The mouth is a functional area, so precision and conservative dosing are especially important. Temporary side effects can include mild bruising, tenderness, asymmetry, or difficulty with movements such as drinking through a straw, whistling, pronouncing certain sounds, or maintaining a firm lip seal.

These effects generally improve as the neuromodulator wears off. Unlike hyaluronic acid filler, a neuromodulator cannot be dissolved after treatment. Patients must allow its effect to fade naturally.
