What Is a Lip Flip? How It Compares to Lip Filler

Patients who want a little more upper lip show often wonder whether they need a lip flip or lip filler. Although both can enhance the appearance of the lips, they work in completely different ways.

A lip flip uses a very small amount of neuromodulator to relax part of the muscle surrounding the upper lip, allowing the lip to roll slightly outward. Lip filler, by contrast, places hyaluronic acid directly into or around the lips to add physical volume, improve definition, and refine shape. Understanding that distinction is important because the right treatment depends on what you actually want to change.

What Is a Lip Flip?

A lip flip is a nonsurgical injectable treatment in which a small amount of a neuromodulator, such as Botox®, Dysport®, Jeuveau®, or Daxxify®, is strategically injected into the orbicularis oris, the circular muscle surrounding the mouth.

The orbicularis oris is involved in speaking, eating, drinking, smiling, and pursing the lips. By temporarily reducing muscle activity in carefully selected areas of the upper lip, a lip flip can allow the lip to relax and turn outward slightly.

The treatment does not make the lip physically larger. Instead, it may reveal more of the pink portion of the upper lip, particularly when smiling.

A lip flip may be appropriate for someone who likes the natural volume of their lips but feels that the upper lip disappears, curls inward, or looks thinner when they smile.

Because neuromodulators are FDA approved for specific cosmetic indications rather than treatment of the upper lip, a lip flip is considered an off-label use.

How Is Lip Filler Different?

Lip filler changes the lips by adding material rather than relaxing a muscle.

Hyaluronic acid is a substance naturally present in the body that has an ability to attract and retain water.   • Restore volume that has diminished with age
  • Improve the definition of the lip border
  • Refine lip shape and symmetry

Lip Flip vs. Lip Filler: Which Creates More Volume?

Lip filler creates actual volume, while a lip flip does not.

This is probably the most important distinction between the two treatments.

If you want noticeably fuller lips, greater projection, or restoration of volume that has been lost over time, filler is generally the more appropriate treatment. Filler can also be placed with considerable precision, allowing Dr. West to address the shape and proportions of the lips rather than simply making them larger.

A lip flip is much more subtle. Because it changes muscle activity rather than adding volume, the improvement comes from allowing a little more of the existing upper lip to remain visible.

For someone who already has adequate lip volume but loses upper lip show when smiling, that subtle change may be exactly what is needed.

Which Treatment Looks More Natural?

Either treatment can look natural when it is performed conservatively and selected for the right patient.

An unnatural result is generally not an unavoidable consequence of lip filler. It is often related to excessive volume, inappropriate product selection, poor placement, or treatment that does not respect the patient’s existing facial proportions.

Dr. West has long favored a conservative approach to lip enhancement. Rather than treating every patient according to the same proportions or injection pattern, she considers the lips in relation to the rest of the face. Her approach to dermal fillers emphasizes using minimal material to restore or enhance volume while maintaining a natural appearance.

The same principle applies to a lip flip. Too much neuromodulator around the mouth can interfere with normal movement, so precise placement and conservative dosing are particularly important.

How Long Does a Lip Flip Last Compared with Lip Filler?

A lip flip generally does not last as long as lip filler.

Since only a miniscule amount of neuromodulator can be used around the upper lip, some patients notice their lip flip beginning to fade after approximately six to eight weeks, while others may retain a subtle effect for two to three months.

Hyaluronic acid lip filler lasts considerably longer. Results may last approximately nine to twelve months, although duration varies according to the filler selected, the amount used, the patient’s metabolism, and individual anatomy.

Can You Combine a Lip Flip with Lip Filler?

Yes. For some patients, the two treatments can complement one another because they address different aspects of the lips.

A small amount of filler may be used to improve volume, structure, or definition, while a conservative lip flip can change how the upper lip moves or how much of it remains visible when smiling.

That does not mean everyone benefits from combining the two. Some patients need only filler, some may prefer the subtle effect of a lip flip alone, and others may not need either treatment.

What Are the Trade-Offs of a Lip Flip?

The mouth is one of the most active and functionally important areas of the face. Weakening the orbicularis oris too much can temporarily affect activities such as drinking through a straw, whistling, speaking certain sounds, or maintaining a firm lip seal.

Lip filler has a different set of considerations. Temporary swelling, tenderness, and bruising are common following treatment, particularly during the first several days. As with any injectable procedure, proper product selection, anatomical knowledge, and injection technique are essential.

Which Is Right for You: A Lip Flip or Lip Filler?

If you mainly want a little more upper lip visibility without adding volume, a lip flip may be worth considering. If you want greater fullness, improved shape, more definition, or restoration of age-related volume loss, lip filler generally offers more control over those changes.

The distinction may sound simple, but lip anatomy varies considerably from person to person. The most natural result comes from identifying what is actually creating the concern before choosing the treatment.
